Request for Reinstatement

Use this form to request to be reinstated in your degree or certificate program after having sat out for the required length of time. You will need to meet with your advisor and develop an academic plan for success before being reinstated.

The deadline for filing the form is the Friday before the first day of instruction as published in the for the semester in which you want to re-enroll in your program. 

Note that, if you took classes at another college or university during the “sit-out” period, you no longer qualify to request reinstatement. To request readmission as a degree-seeking student, submit an application for admission and an official transcript from the other institution to the Office of Admissions.

 


 

Appeal of Academic Disqualification

Use this form to appeal your disqualification. You must provide a written appeal and see your advisor to develop an academic plan for success. Your request will be routed first to the dean of the college or school of your major, then to the appeals committee for final approval. The decision of the committee is final and may not be appealed further.

You may only file an appeal after you’ve been disqualified and before the Friday before the first day of instruction as published in the . If you don’t appeal or if you miss the deadline, you can request reinstatement after you’ve sat out the required length of time.

 


 

Application to Associate/Certificate Program

Use this form to request admission to an associate, certificate, or occupational endorsement program if you’ve been disqualified from a bachelor’s degree program. You will need to meet with your advisor (in the new associate or certificate department) and develop an academic plan for success. If you’re not sure who that advisor will be, see “Academic Advisor information” at the bottom of this page. The appropriate dean will then review your application and your academic plan for success and will make the final decision.

You can file this application as soon as you’ve received notification that you were disqualified from a bachelor’s program, but you can also file it for a later semester. The deadline to use this option is two years from the date you were disqualified.

If you are admitted to an associate or certificate program, and you later wish to return to a bachelor’s program, you will need to apply for admission through the Office of Admissions.
